As someone who's worked around heavy machinery and lived close to a coconut farm, "Left, right, up, down, forward and back" is a habit I still carry to this day

Truck driver here. Whether I'm driving, walking around my yard, or a customer's yard, I keep my head turning and my eyes moving! I look like a bobblehead, but I've never been run over.

I grew up on a farm. You always make eye contact with the operator BEFORE approaching. Same with aircraft and helicopters.

When I met my wife, I was driving small forklifts in a warehouse, and she wouldn't get within 30 feet of one of the things so long as they were on, they didn't even have to be moving. You could say it's a little excessive but you can never say she was in an unsafe position around any forklift or its operator.

Happened to a guy working with my dad on the railways. Coupling up a three car unit. Driver couldn't see him, started to roll forward, pushed the guy over and rolled him up underneath the engine. Dead before the emergency services could get there, before even the railways emergency response team could get there.

There followed a safety campaign "If you can't see me, I can't see you" - "Be seen - Be safe".

I've been around mining equipment and workers for the past 20 years. These guys would tell me stories of mining accidents all the time. One that stuck out was similar to this. He was pancaked and no one really noticed till the next run through the area. We don't even make a speed bump for these things.

A good lesson I learned working in a warehouse: machinery has the right of way. Mostly because peds can stop on a dime, but heavy machinery can't.
