I think alot of it comes down to the fact that the item demands that you have both attackspeed and Health. Very few champions want both stats and those that do... would rather just build attackspeed and health. Instead, the item appeals to champions that already have attackspeed (urgot and briar) but want to stack a good amount of health.

you need to be a champion who stacks ALOT of hp AND auto attacks alot. Its just got 2 conditions that alot of champions dont fill very well.

The damage on Titanic also got heavily nerfed. I remember buying Titanic as the only HP item in my build and the active would clear the whole wave and do so much damage. Right now it feels super underwhelming and I actually didn't know about the nerf until I bought it myself and went "Wait, did they nerf this?" My friend confirmed that they did. I would casually build the item on assassins, ADCs, the active damage was just too much to skip out on.

The part about abusing the item is true, but there's a 2nd reason you don't see it often, and that's because most of the champions that like Titanic are immobile melees that love to get lots of health in their build. Those same champions *hugely* value Stridebreaker's active for helping them actually be able to interact with ranged champions. So because Tiamat is a component of both items and thus you're only allowed to have one of the, Stridebreaker is chosen over Titanic by them every time (look at Trundle for a prime example).

Bel'Veth would synergize with it very well, but the active gets nerfed way harder than other on hit items on Bel'Veth. Her basic attacks doing reduced damage gets counteracted by her super high attack speed, but it makes items that grant a bonus to one attack like Voltic and Titanic worse on her than they would be, because her attack speed doesn't make up for her reduced damage with Voltic attacks and Titanic active.

I think its because stridebreaker is just better. The item would need to be like 700 or 800 hp to actually compete with how strong stridebreaker is.

You need to like health (stacking) and AD, which limits it to some bruisers and some tanks. You need to be autoattacking a lot, which rules out some bruisers and most tanks. You also need to care about the additional waveclear to some degree, and preferably also the autoattack reset.

So yeah, it's niche for sure.

A lot of chanps have decent synergy with the item, the issue is that they tend to be better with the other tiamats. I feel like it needs it's bonus hp to ad passive back, because that would make it stand out amongst the other tiamats

Funnily enough even Urgot players don't generally build it either, extra waveclear is not needed, damage is nerfed on ranged, and by the time he completes his best first item(BC), he must build tank or else he gets blown up in teamfights.

Champions that build it want the health and AD of course, but the champions that build it either have very strong synergy with an auto attack cancel (Darius), or have innate attack speed so they proc the on-hit a lot (Sett, Urgot, Briar), or you're Sion and you get hilarious amounts of AD from it

I feel like it should be a more niche item pick though. While its a pretty big buy on briar and urgot because they have built in attack speed its also a great matchup item pick for toplane bruisers that need waveclear.
