Wax is nothing but cerumen. Cerumen is the secretion of the ceruminous glands just like the sweat glands in the ear canal a person with an oily skin can secrete lot of ear wax which is wet in nature and over a period of time this gradually accumulates and leads to blockage and resulting in temporary conductive hearing loss. A person can also form hard wax and it can get deeply impacted causing pain when it presses on the eardrum. There is also a condition called Keratosis Obturans where the expanded wax expand the ear canal and eat away the skin of the ear canal causing extreme pain and pressure on the tympanic membrane. So a simple ear wax can be periodically cleaned by the doctor the symptoms of ear wax accumulation being hard of hearing which is progressive in nature and if the patient has used buds, pins, hairpins, a pen tip Etc and it has resulted in pain and further blockage that most probably means he has pushed the wax which was outside further deeper and caused damage to the eardrum. So it is safest for the patient to approach the ENT doctor after softening it with so many softening agents even simple olive oil drops can be used to soften the wax periodically and then they can visit the doctor for ear cleaning the safest and best method for us to clean the ear is your syringing using sterile instruments and normal saline with appropriate a simple syringe. We can soften the ear wax for some time with a dilute hydrogen peroxide drops instilled for some time and then when you syringe the safely the ear wax comes out since we are ENT surgeons and we know that anatomy perfectly we will be doing it very safely and painlessly. Therefore self-medication and self-cleaning is not at all advisable when the wax buildup is too much in case it is a liquid wax we can do suction clearance also under the operating microscope. If the condition is a Keratosis Obturans after sufficient softening of the Keratosis under microscope you have to give local anesthesia and carefully with gentle micro surgical ear instruments we have to remove the entire Keratotic mass so that no trauma happens to the eardrum and patient starts hearing immediately.
