Great questions! As it turns out we have the ability to make bacteria regulation certain genes but we don't use regular genes so the insulin is translated constantly. Also, we insert a gene that makes the bacteria antibiotic resistant. We culture this bacteria in large vats in the presence of antibiotics (only bacteria with the antibiotic resistance gene will survive.) As long as the bacteria is "fed" it will produce insulin. Isolating insulin from the bacteria is a simple centrifugal process.
