Hepatitis B virus infection causes chronic hepatitis B which leads to liver damage in the long term. Hepatitis B virus is responsible for causing self-resolving hepatitis , chronic hepatitis , chronic liver disease , cirrhosis and liver cancer. All these conditions can be essentially prevented if hepatitis B virus can be diagnosed and treated early.
A huge population across the country are infected by hepatitis B virus and are not even aware of this condition, let alone seek treatment for it.
Currently the scourge of hepatitis B has been very well controlled at all stages of disease. I discuss these hepatitis-B related issues in this video.
Two major viruses causing Hepatitis
Hepatitis B & Hepatitis C
Hepatitis A , Hepatitis B can also affect the liver but they do not cause as much damage , we will discuss them later
Happens due to hepatitis B virus
296 million worldwide
1.5 million new infection every year
8.9 lac deaths in 2019
According to WHO estimates only 10% of the infected population were aware of their hepatitis B infection and only a quarter of those were on active treatment
15-25% of CHB patient will develop potentially life threatening complications like cirrhosis , liver failure or liver cancer
Major reason for liver failure and mortality earlier
It’s not a disease like heart attack or brain stroke that causes death immediately.
It develops slowly over years but eventually can be fatal
Now survival improve due to availability of vaccination , medications for cure and transplant
Major source of infection is transmission through blood .
Mother to child is very common . Hence all pregnant mothers are screened for virus . The newborn child is at risk hence needs to be managed properly to prevent infection.
Vaccination available for cure at 0,1,6 months
Part of the Universal immunisation program .
All children should be vaccinated .
Adults can also contract infection hence should be screened and vaccinated if not already done.
All adults are not routinely screened for Hepatitis B virus in India
However, My suggestion would be to get screened for the virus whenever you next visit a clinic or hospital or are getting a any blood investigation done .
Timely identification and management can effectively prevent chronic hepatitis B related Cirrhosis .
Liver cancer can also develop due to the virus . There is increases risk of liver cancer in every patient of cirrhosis however in patients with hepatitis B , cancer can develop without cirrhosis. So it’s important to look for cancer in such patients.
Diagnosis depends on looking for Hepatitis B antigen (HbsAg ) and then confirmation by count the virus copies in the body( HBV-DNA - PCR )
Your doctor might ask you to regularly follow-up with Liver functions tests (LFT), Ultrasound of the abdomen , HBV-DNA-PCR and alphafetoprotein (AFP) every 3-6 months
Chronic hepatitis B used to be a fatal infection a few years back but not any longer. Very good medications are available now that can virtually eliminate the virus from the body. Even better medications are expected to be available in the future.
So remember
-Get screened for virus at the first opportunity that you get
-If you are infected get your family also screened
-Please follow the surveillance protocol as recommended by your doctor.
-Better and more effective treatment are available at all stages of the disease hence be compliant with your follow up and treatment.
-Better treatment is expected to be available in the future so keep fighting the battle.
