Cystitis vs asymptomatic bacteriuria during pregnancy

Asymptomatic bacteriuria:
Asymptomatic patient with positive urine culture (≥100,000 CFUs/mL) .
Screening usually performed at 12-16 weeks gestation
Treatment reduces progression to UTI, pyelonephritis & complications (eg, preterm birth, low birth weight)
Acute cystitis:
Symptomatic patient (eg, dysuria, urgency) with positive urine culture
Considered a complicated UTI
Treatment / management: .
Amoxicillin-clavulanate 5-7 days OR
Cephalexin for 5-7 days OR
Fosfomycin as a single dose OR
Nitrofurantoin for 5-7 days (avoid in 1st trimester & at term)
No fluoroquinolones in any trimester
No trimethoprim-sulfamethoxazole in 1st trimester or at term
