Honestly I like the idea of the refs treating every match like a shoot. "Oh you didn't kick out? Sucks to be you." Keeps the writers, and wrestlers on their toes, and makes for a more unpredictable product...when it happens. Plus it's never the ref's fault that someone doesn't kick out.

I think counting someone out unintentionally is a good way to start a storyline or even acknowledge it and admit the ref fucked up just like they do in football, basketball, etc. It would definitely lend an element of realism.
