Thank you, thank you, thank you!!! I am a new graduate nurse and I am finding it really hard to give reports to other nurses. I believe this template can also be really useful to ask pertinent questions to nurses who will be giving me reports on patients. As term cna id give report to cna s and rn. The main issues I had were the ones who hung out else where and couldn’t be bothered to want in the common area or find me for report. As we were supposed to be.
No I’m in a family practice. What very different dynamic. As it still has a report system going from rooming to reporting to the provider. It has its own things that are important very much like what you’re talking about.

As a nursing professor and RN for 40 years, I suggest you devote 30 seconds to the HxPI (history of present illness). (Think more like a doctor). Describe the "STORY" that brought them in, their signs/symptoms, and time frame (often missed). Example: "In Room 2, I have John Smith, length of stay 4 days. He presented to the with his son from home c/o'g of a 3 day history of low grade fever, nausea, vomiting coffee ground emesis, and abdominal pain...." Also, your report should include the work-up in the ED. What did they do and what did it show. Too often my students miss the entire beginning of the pt's hospitalization. Reading the ED physician's notes and Hospitalist's H&P will reveal all the important stuff (including the initial work-up and diagnostics). It will also spell out the admission PLAN OF CARE so that nursing and medicine are on the same page. Armed with that insight and perspective, everything else will fall into place.

Im having hard time giving hand off report as a foreign nurse and English is not my primary language. I was so nervous yesterday giving report to the incoming nurse…felt like im crying inside coz i dont know whats the step by step i need to tell..my preceptor didn’t help me even backing up some details i missed…😢😢😢 thank you i saw ur video
