4x soul sand
1x pickaxe
1x sword
1x bow
20x arrows

to be honest though in a real 1v1 with no limitations, the wither wouldn't even take damage because it can fly.

edit: this comment was made before the warden got its sonic boom ability, back then it was only melee.
