It was all a setup by the heavy. Notice how when the heavy was dead he wasn't and everyone was just playing along while the engineer was thinking he got away with it. Then how does it end? The heavy comes out as the only survivor.

So what does this mean you ask? The heavy was actually a lord of change setting up a show to please his master.

0:01 the red heavy walks in enthusiastically to his next payload match, he sings to himself. what a good day to be alive!
0:04 the most iconic event takes place, blu engineer pulls the trigger of an unusual telescope looking object, the heavy soon knows that his time has come
0:10 red spy discovers the scene of the crime, realizes the heavy has fallen
0:17 red sniper runs into the red spy and the only other witness: blu engi. he soon also notices the heavy has been killed.
0:31 red spy vows and swears that he will be the one who puts the murderer behind bars
0:45 red soldier, after making his grand entrance, offers his help to the red team and blu engie, and finds a strange weapon that lays on the ground. he makes one vital statement the team will need for the rest of the case: the heavy has died.
0:58 red medic comes in and rushes into the crime scene, and attempting to resurrect the heavy with a delicate operation to the face, he fails.
1:12 although after an UNsuccessful procedure, he managed to deliver delicate information to the rest of the red team, noting that the heavy has been indeed, murdered.
1:19 the clueless red scout marches onto the scene, and after a long, restrained breath of disappointment from the red spy, he was warned not to return to the crime scene ever again.
1:33 somehow, red heavy has gotten back to the scene and examines his cold, dead body. he reminds the team about soldier's information, that he was dead. he ponders, what disgraceful, inhumane being would do such a thing to murder him.
1:39 red demo makes his appearance by taking another gulp out of his bottle o' scrumpy. he claims he was the murderer, he has attempted to kill heavy, and proves it by red sniper meeting his fate by a bullet to the head.
1:54 once the team realized that red demoman allegedly assassinated the heavy, he immediately says it was a failed assassination and that blu engie was the one who has successfully put his life to an end instead.
2:14 the blu engineer knows he was found out to be the culprit, and trying to hold it back in his throat, he finally gives in and admits he indeed, murdered heavy. the angered red heavy lashes out at blu engineer for trying to disgracefully kill him, while the red spy was just about to witness what was about to happen.
2:28 the heavy, with one last shaky breath, pulls out a pistol made from flesh, and out of anger, blasts the blu engi in the skull. red spy also passes away from depressing circumstances, trying to cope with the trauma.
2:41 heavy triumphs at how he managed to get back at the engineer, and that he's safe and alive. but after he takes a final glance at the scene, he soon realized, he was doing something so hypocritical, so vile, and something so stupid.
