I watched the anime. I have mistaken .As she's going into cardiac arrest in this scene.She passed away during her surgery and Kousei's performance.
One last duet was sad and cheerful at the same time because it wasn't like when Kousei's mom, Saki pass away.Kousei moved forward and began playing with Kaori one last time before saying his goodbye as Kaori fades away
