It shows mission failures are not a failure of one person its a chain of misunderstanding and misintepretation makes any mission fail. But the last person who finish it gets all the blames.
And the first person who run away from the blame is manager.and the chain goes till worker. This should change.according to me its a team work. If there is a failure whole team should be responsible to accept their faults. If its success the success should be shared and celebrated. Without partiality.
