Pace and Ablate for Afib

Afib is a disorder of both heart rhythm but also heart rate. When the rate is fast then the patient is generally more symptomatic. In addition an excessively fast heart rate, if lieft unchecked, can cause the heart to weaken over a period of time. Medications to control the heart rate can often cause unacceptable side effects. In this video, I discuss an interesting technique called 'Pace and Ablate' which can control the AF without the need for medications.

We are a doctor couple, my wife has been having AFib for several years now, she is on betalockers, flecarite and anti coagulants for several years. Despite treatment her episodes of AFib attacks were increasing and we were seriously considering ablation.
She then seriously took up yoga ‘pranayam’ ( breathing exercises) which she believes decreases the sympathetic tone, she also worked to reduce the ‘ triggering’ points which in her case was tea, coffee, heavy meals and stress.
Though she still takes regular medication, but those seem to be working now, the episodes are generally not happening and if she feels her heart rate is increasing on her apple watch she takes extra dose of Flecarite, she is also sleeping well due to breathing exercises.
I thought I should share, it has worked for her and may do for others

I would just like to say thank you so much for a very informative and easily explained description of Pace and Ablate.
I have actually had this done to control my troublesome aFib.
I could not tolerate even a small amount of beta blockers and flecanide did not work at all. I'd had a chemical cardioversion (3x bolus administrations, increased each time) which only made my heart beat even faster.
After the cardiologist explained the offered PVI ablation had about a 70% chance of success i decided to go the pace and ablate route.
I have never looked back and now, nearly a year on from pacer fitting, my AV mode ablation and a few pacemaker tweaks, my quality of life is so much better and I am able to exert myself cardiovascularly as I used to. I am so grateful.

Dr. Gupta I have been dealing with afib for 9 years and at 65 years of age it is getting worse. I am now on max dosage for betablokers. It is still very unsettling. I listen to your calm voice which helps me during my episodes. Please continue to share. Your knowledge and insights.

Thank you Sanjay. Now 81 and still very active, I have recently had a pacemaker implanted and due for the ablate to follow shortly. You have certainly cheered me with your very helpful summary. It will be good to be free of the pills which do give rise to side effects. You always describe matters so clearly……much appreciated.

I’m an RN in America and received a pacemaker and AV node ablation last week for tachycardia 135-140. Before the procedure my husband and I were surprised that the cardiac RN told us we had the best questions of any patient. I attribute this to watching your very clear and well paced YouTube videos. Thanks so much from across the pond. ❤

Thank you so much for for explaining this in such detail. I have AFId and have been fitted with a CRD pacemaker and undergone a AV node ablation to correct a small amout of heart block and a slightly low left ventricular ejection fraction along with a fast heart rate. There are so many references to this type of heart correction that state a much shorter life expectancy! Hearing your comments on this corrective measure have put my mind at rest. Thank you so much.

Had AV node ablated and dual chamber pacemaker implanted last week. Feeling so much better and sleeping better with my heart at a slower steady beat.

Thank you so much for your informative videos. Earlier this year my general cardiologist recommended a Pace and Ablate procedure for me. It wasn't explained to me nearly as well as you did in your video. Luckily, the Electrophysiologist I consulted with didn't feel it was indicated at this time and instead, performed a more traditional ablation procedure, the third one I've had in seven years. I'm happy to say that so far I've remained in normal sinus rhythm. I realize that nothing lasts forever and that the possibility of a pacemaker still remains in my future. Meanwhile, I take my meds, monitor my rate and rhythm, and do my best to live a healthy lifestyle. Your videos give my great peace of mind and reassurance. Keep up the good work.

I am scheduled for a pacemaker due to a racing irregular heart beat. My cardiologist had explained the procedure but this video has put my mind more at ease.

This was so helpful to me! My mother is 86 and having this procedure done in a few weeks. I'm very nervous for her but this gave me a better understanding. I'm hoping this can still help her have a better quality of life. Thank you so much!

Thanks, Dr Gupta for this informative and clear explanation of Pace and Ablate. I found your clarification of what you call the 'escape' mechanism very reassuring. I am listed for Pace and Ablate this summer in Nottingham and was worried a good deal about the "What scenario. Knowing that my heart won't actually stop but continue beating, albeit very slowly is, as I say, very reassuring. You don't need me to tell you this, but you really are an excellent communicator!

I am having pacemaker inserted in four days time and 2 or 3 months later ablate with Dr Affolter in Aberdeen. That was a stunning, clear and brilliantly pitched presentation. You answered all of my underlying questions. Thank you SO much.

Thanks for the explanation. I had an A/V node ablation and a pacemaker inserted back in 2017. I understood some of the procedure. Was so scared at first, but was glad I had it done. Could not tolerate many of the cardiac meds. I was diagnosed with A-Fib with RVR.
