It's neither. It depends on how "good" your character has been in her presence up to this point. Notice that in Act 1, Gale will comment on certain things you do, like standing in front of a loaded crossbow to save a goblin, or freeing Arabella. Shadowheart also notices these things. So, if you betray the Tieflings of the Grove, or do other evil acts, she's much more likely to murder Aylin. But if you've been kind and good hearted up to this point, she ALSO decides to do what's right. It all comes down to whether you're a terrible person, or a *good* one. And that honestly is refreshing -- you're being liked by people for your actions, not just because you give them gifts or select the right dialogue options.

I think the important factor here is to have high approval with Shadowheart. @bensummers4732 also suggested not pressuring her into telling you her secrets, instead letting her to come to you, and not letting her have blind faith in Shar. It's also much harder to push her into sparing Nightsong, if you have a good relationship with her, it's easier to remind her of her own doubts (Is this truly what you want?) and letting her decide for herself. I also have no idea if it matters but I also killed Balthazar before finding the Nightsong. In general, my character was supportive of Shadowheart and never shunned her for her Shar worship, but was sometimes skeptical of Shar's methods. I did let her let her do the blood rituals in the Gauntlet and Sanctuary and it didn't change anything for me, so if that is a point towards her staying a Sharran, I did enough to balance that.

I just did this on my honor mode run a few days ago expecting the same result because I had done it before and she killed her so now I have to do the myrkul fight without dame aylin bruh

I have had friends not interfere and she kills her anyway. I think it depends on if you made good guy choices leading up to it.

I didn’t romance her. Spear was in her inventory. I picked the trust option and…..


….she threw awesome the spear, again :D

So seems my character has been a good best friend with 100% approval at that point. ;)

I’ve heard it depends if you leave the Spear of Night in her inventory or not beforehand?

yeah I had high approval, asked her if it was what she truly wanted and she took Aylin out. I save scummed the good result. Next time I just won't pick up the bloody spear
