My #melasma treatment pearls:

🔸 MOST IMPORTANT: Sun protection education. You cannot assume that your patient knows every sun protective practice. Review the importance of WHAT sunscreen to use, HOW often to apply, and other ways to sun protect aside from SPF — wide brim hats, limiting outdoor time during peak sun, and explaining that even a single episode of unprotected sun exposure can reset all melasma progress.

🔸 Review a patient’s medications to ensure they’re not taking something that could worsen their melasma such as a sun-sensitizing medication or certain types of birth control.

🔸 If using hydroquinone (HQ) consider a higher concentration such as 8% or 12%. So often I see melasma that is under-treated. Hit melasma hard and fast and then taper down once you’ve achieved control. HQ can often be compounded with other helpful ingredients like tretinoin, vitamin C, kojic acid, niacinamide, etc.

🔸 Consider oral tranexamic acid. This is probably my all-time favorite melasma fighter because over 85% of patients will see improvement on this medication. Only prescribe if you also feel comfortable performing an appropriate medical screening to ensure that your patient can be on this medication safely.

Amazing transformation! 😮
Seeing a derm in real life is really life changing when you have a more severe skin condition. Even, when it’s really frustrating that you have to see you derm regularly and other people could just wash their faces with dish soap. I‘m on tretinoin for my acne and I only could maintain „perfect skin“ when I go to a medical esthetician one a month.

when using hydroquinone, do you only apply it to the spots affected as a spot treatment or all over the face? not asking about specifically her case since hers was all over her face but just in general. excellent job helping her with her melasma.

When using triluma (I mix Tretinoin, desonide, and hydroquinone 4%- wish I had 8%), what is the length of time you need to refrain after the four months (I was told no more than three) so as to avoid rebound / greater hyperpigmentation?

Hi, can you please answer to this and help me? I have gotten triamcinolone acetonide with azaleic acid for my melasma. I am 24 and have a lot of spots. I am very sad and can't find anywhere if the cream I got does something for anyone with melasma. Does someone know this? I can't use Hydroquinone because I am between skiptype 1-3...

Can you please speak on the brands Eminence, Peach and Lily, Sunday Riley, Drunk Elephant, Indie Lee, and Lancome?? I'm having a hard time picking skin care and I like all of them but I don't know if any are better than the other or worth the money. I'm doing a lot of research but I'd like to know your input!!
