Let us not forget Engineer Expendable who attempted to disconnect the M-5 computer from the ship and ended up with an energy beam up the bum and was vaporised instantly... :P

They don't clearly establish that a death takes place, but the scene where Charlie X removes a woman's face traumatized me as a kid.

The deaths weren't shown on screen, but the disease that killed the crew of the USS Exeter in "The Omega Glory" was pretty messed up.

Decker's death wasn't totally useless after all it showed our heroes how to destroy the Doomsday Machine before it reached any other star systems

Honorable mention to “That Which Survives” poor Ensign Wyatt, Lt. D’Amato and Engineer Watkins all have every cell in their body disrupted “from the inside” by Losira’s touch.

"The Apple" was a three-fer... one poor redshirt killed by a plant throwing poisoned darts, another vaporized (not merely electrocuted, but completely vaporized) by a lightning bolt, and a third killed by an exploding rock. As if that wasn't messed up enough (from the redshirt's point of view), Spock gets hit by the same plant darts and struck by lightning, surviving both, and manages to inadvertently set off one of the exploding rocks without causing harm to himself or anyone else.

The dangerous cave trope even extends to Voyager. The episode when Seska took Voyager, the kazon dropped the whole crew off on a planet with a primitive tribe. One of the crewmen got close to a cave and a big worm creature popped out and OMNOM!

Being crushed after being turned into a cuboctahedron probably wouldn't hurt, but it shocked me as a kid because it happened to a frightened young girl. They averted the "Black Dude Dies First" trope, and I'm sure they meant to.

Wouldn't it have been wicked if the cuboctahedron had bled when Rojan crushed it? Maybe they should have packed it with chicken guts, too. That would've been cool.

Crewman Darnell was an early role played by legendary soaps actor Michael Zaslow. Who held down roles in Guiding Light and One Life to Live. Going full circle, one of his last roles was an appearance in Star Trek First Contact. He passed away in 1998 from Amyotrophic lateral sclerosis.

Actually Commodore Decker's death was not pointless it was heroic. His sacrifice did register on the ship's instruments and so gave the idea to Capt. Kirk to do the same with the USS Constellation.

You missed Lt. Galway dying of old age in the "Deadly Years", and you completely missed "Wolf in The Fold" where Lt. Tracy was murdered by Redjac.

Back in the 90's there was a sketch comedy show called "The News" (basically a discount version of Saturday Night Live) that had one good sketch about the plight of the Enterprise's last remaining red shirt as he desperately tried to avoid an away mission.
