As a native English speaker I think I'd prefer to hear and say
"Sorry, I didn't hear you"
"What, I didn't hear you?"
"wait what?"

As long as you say them nicely it's fine

In America, usually when we say “I beg your pardon” it means that the person you are conversing with might have said something deeply offensive or rude, and it is almost provoking them, or daring them to repeat their insult.

In America I often just ask, “Say again?” Or “Can you repeat that?”

You are very formal for almost all of your content, and grammatically correct of course, but some of your tips are a little stiff for coaching people for casual conversation. Do you ask natives of the country how they would do something, or is it generally a reflection of your own observations and experiences? If you are primarily interacting with people in a customer service context (shopping, eating at a restaurant, etc) then people will be using their own most formal language. They would speak very differently “off the clock, ” meaning anywhere outside of the workplace.

"Excuse me?" can easily be read as accusatory, at least in the US.

These are awfully formal. In Britain these might work, but in America we are often less formal. It does depend on the context. If you need someone to repeat something some less formal options are:
1.What did you say?
2. I missed that. Could you repeat yourself?
3. Could say that again?

Those are just some. Formal is not always better. Sometimes being informal puts people at ease and allows them to be more receptive to ideas. My biggest tip to anyone trying to learn English is that it does not have consistent rules for anything. Any rule you find always has counterexamples and lots of them. Sometimes if you are lucky you will find consistent rules, but learning English is mostly just a lot of memorization.

So in my experience if an American says "I beg your pardon". They heard what you said and what you said is unacceptable.

As the only context I've actually heard it used in and used it in myself. Is the "I did not just heard what I think I heard."

Yeah still means repeat what you said but with an emphasis on "if you said what I think I know you said we are going to have a problem." You're going to be getting a piece of my mind.

It's a bit of a warning. That you might want to back track on what you just said.

Sometimes, here in Virginia, I hear people saying “Could you say that again?”, “Can you say that again?”, “Say that again?”

"I beg your pardon" is used in the U.S. when offended. Someone says something bad to you and even if you hear it, you might say "I beg your pardon?" Sort of like saying "What did you just say to me?"
