I believe he loses to Douma purely because Akaza’s skill relies on the fighting spirit of the enemy, which, Douma lacks because of his psychopathy, he lacks all emotions, therefore he cannot predict Douma using his senses/demon art.

It wasn’t just Akaza’s morals for eating certain people to cause him losing against Doma; it was also Doma’s Blood Demon Art (BDA). (Spoilers ahead for Doma’s BDA). Doma’s BDA allows him to freeze a large area around him and spread it farther with his fans and he is able to create things from ice. His most deadly ability from his BDA is the air is so frozen around him that microscopic icicles form making it deadly for anyone who breathes them in which cuts up the opponents lungs. This is primarily what caused Akaza to lose due to the limited breathing which in turn limited his power in that fight. Akaza has even voiced out that Doma’s BDA is basically cheating and that he should fight him with only physical attacks. This is also why Akaza prefers to fight Kokushibo since he doesn’t cheat in Akaza’s eyes and fights with only honor and mainly physical attacks.

I think losing your morals helps you quite a bit in anime-

All of this theory is thrown out the window when you realize Douma counters perfectly every ability Akaza has, including his compass needle.
Him eating women won't change his position in the ranks one bit XDD

Respects women and children even when he is a demon AND upper moon.
With only eating man and always wanting to fight i respect him a lot.
Even though eating men is weird it gives us hope that even a demon that has been a demon for years still have good in theyr heart.
