My theory is that RDJ's Doctor Doom isn't Tony Stark, but just Victor Von Doom in Tony Stark's body.

In the What If...? comic titled "Demon In An Armor, " Tony Stark was Doom's college roommate instead of Reed Richards. Victor body swaps with Tony to steal his identity and resources to further his goals. They could have a mix of that story's Doom and the God Emperor Doom from the Secret Wars comic run be the villain in Doomsday and Secret Wars. Tony Stark's iconic line from Superior Iron Man "it's easy play god when you're the smartest man in the world. All this time, I've been playing human" goes really well as a line for God Emperor Victor Von Doom, and could be a cool nod to this Victor technically being an evil Tony.

From what was confirmed at the reveal of RDJ's Doom, he is Victor Von Doom, and is from another universe, based on Kevin's comment about how this shows anything can happen within the multiverse. It all lines up perfectly.

In addition, if they do go this route, they could ultimately hype up the actual main universe Doctor Doom by having the heroes lose in both Avengers Doomsday and Secret Wars, only to have a non-RDJ Doctor Doom come out of nowhere and kill RDJ's God Emperor Doom at the end. It'd be a good way to show how powerful this new main universe Doom is, since he just easily wiped out what's supposed to be the most powerful version of Doom, while also fitting well with his character. Perhaps Main Doctor Doom had plans of overthrowing the baron of Latveria, and RDJ's God Emperor Doom's actions had a ripple effect where it made normal Doom too much of a high profile, making his overthrowing more difficult, now that the world knows of his existence. Main Doom kills RDJ Doom for getting in his way, and the only reason the heroes are still alive are because Doom saving the day for his own means, not for the greator good. This would make MCU Doom an overlooming threat like Thanos was, except that the heroes know of his existence, but aren't going after him due to his raw power and because he saved their lives.
