Diaphragm-Sparing Blocks for Shoulder Analgesia

Which nerve blocks you should use for analgesia in patients with history of respiratory disease? Phrenic nerve block and diaphragmatic paralysis with Interscalene Block are challenges that we all face in everyday practice of nerve blocks. We all often take care of patients who would be beneficiary of interscalene analgesia. However, the presence of respiratory disease makes it a uncomfortable to proceed with an interscalene brachial plexus block because of the possibility of diaphragmatic paralysis. NYSORA developed several algorithms in its COMENDIUM of Regional Anesthesia to assist the clinical decision making in this clinical scenario. This video outlines the principles of shoulder analgesia in patients with respiratory problems.
