In Revenge of the Sith, Palpatine’s deflected Force lightning causes his face to become deformed. Many have speculated that this was his true form, which he hid with Sith alchemy. The Revenge of the Sith novelization - no longer official canon, but still insightful - says that, under the pressure of his electric hatred deflected back on him by Mace Windu, “his skin flowed like oil … as though even the bones of his skull were softening.”
So while it is an interesting theory that he hid his appearance, it is not actually true. What is even more interesting is the fact that after announcing the attack by the Jedi that left him scarred and deformed, he still presented a normal image of himself to the galaxy. Broadcasts of him on the HoloNet digitally altered his appearance, showing no sign of his deformity. And Imperial propaganda pieces showed no indication of it either.
I guess the grandfatherly appearance he presented throughout the Clone Wars was important enough to carry into the Galactic Empire.
