I can't fucking believe I've spend around 3-4 hours in the divine tower of caelid and never tried going right instead of left 😑

The divine towers form the Elden Ring, surrounding the Erdtree. Each tower is dedicated to a demi-god who possesses one of the shards of the Elden Ring. Since the Shattering, the Ring had gone and each demi-god has left it's tower. When you bring a Great Rune back to the tower, you are essentially returning the shard of the Elden Ring to it's correct location. If that all sounds a bit weird and it does not jive with what you experience in game, remember than each demo-god is a metaphysical entity, so the form you fight is not their only form. Malenia is the easiest example because she is both the Blade of Miquella and the Goddess of Rot. Miquella and Malenia are twins - Miquella is healing while Malenia is decay, or life and death. The Blade of Miquella ends life, and death has never known defeat - no-one escapes death. The demi-gods are both concepts and bosses. In the same way, the Elden Ring is both a ring and a circle of divine towers around the Erdtree. The thing in the middle of the divine towers is the Erdtree itself. I know it doesn't look like that on the map, particularly since you can enter the Erdtree in Leyndell, but if you look at it from another position the base of the tree is closer to the center of the circle and the tree leans towards Leyndell. (Personally, I think when the story was first imagined, the tree was in the center of the sea surrounded by towers but you could also get to the tree from Leyndell. When it came to trying to create that in game, it wasn't so easy. )

8:20 Fell Twins are next to Rykard being the coolest fights that are very easy. With Fell Twins, the fighting space is infinite. Took too much damage? Ruck backwards and heal. One pretty much left me be while I fought the other one.

in my case the chest at the "Tower of Return" which should lead you to the divine tower (for the rune of malenia) first brought me to the grace "Divine Bridge" in Leyndell (ashen capital)
After beating that giant armed with a halberd, on the left I found an active portal, entering it led me to the "isolated divine tower"

To open all towers you have to fight all the bosses for the runes.

There is also a boss fight (Godskin Apostle) at the very bottom in the divine tower of Caelid

Cloud in the middle is likely DLC area that is a floating city upon the clouds which are solid and you can walk on. In order to get there you will need a glider, likely made available by DLC.
