The mother has to be on treatment because the transmission from a mother to child happens at 3 stages, that is when she is pregnant and the baby is in the womb, then when the baby is delivered, at the time of delivery there is a risk of transmission and after delivery, during breast feeding. So these 3 stages can be brought down to zero percentage of transmission if the mother is on appropriate treatment. So this very important that the mother is taking the right treatment and if her Viral Load is undetectable. If her viral load is undetectable, if there is no risk of transmission to the unborn baby, during pregnancy or even at the time of delivery and she can breast feed like any other mother who is positive or negative.
