I work in Echo, one of the first manifestations of a pericardial fusion is the collapse of the right atrium. Also, this would be seen behind the heart in a two chamber image. Unless the effusion is loculated, but then they would be discussing something completely different. Once again, a medical show throws out a bunch of terminology but doesn't use it correctly.
