I think I figured out the enemy scaling mechanics. I think the type of enemy scales based on the area you are in, for example charred feral ghouls in watoga, and regular feral ghouls in other places. As for level scaling, certain types of certain enemies can be higher level than others, for example scorched annihilators can be level 100. Also, if a high level player and a low level player are fighting the same enemy, the enemy will appear as low level for the low level player, and high level for the high level player. The enemy will do damage and take damage to and from the low level player as if it were low level. It will do damage and take damage to and from the high level player as if it were high level.
