MRI – UNILATERAL HIP

In this video I tried 3 different methods for better unilateral hip imaging. Coil combination and patient positioning are one of the key for having good images. I will also share my thoughts on which methods I find as the best. Tune in and join me.

Thanks for watching, following and support.

Our department always does a bilateral hips in the coronal plane (T1 & STIRS) so we use body coil with the spine coils. Then we do unilateral sequences with the same coils.

I normally use the flex coil and try to wrap it closer to that hip than the other. I don't completely wrap the hip. The patient is lying on the spine coil and when I snap in the flex coil I pull it over so the affected side has the coil wrapping down to meet the spine coil. Images always look great. We have to do exams in 15 minutes so there isn't time to experiment with other positions.

Nice one Bac! yes, i dont think its too much waste of time to perform the 2 bilaterals in protocol (t1, stir) and then reposition for the unilateral sequences if theres benefit. Also, it can be helpful for arthroMR for when the patient comes back from getting the contrast injected. Thanks for sharing.

Lovely as always. For bilateral and unilateral hip I use the body coil over the pelvis in combination with spine coil. Using the same coil configuration I'd scan a unilateral hip if needed. But on my scanner using the flex coils take a little longer.

Bac thanks for the video you are supermriman 💪😎
Pls show how to do Siemens mri machine quiet?)

We always do unilateral. We throw in a bilat coronal stir through the entire boney pel.

Unilateral hip ultraflex 18 for normal size patients . Obese or larger patients 18 channel body coil. Morbidly obese patients body 18 on top ( not wrapped ) in combination with spine coils

Good video as usual Bac.

Dr rarely do just a side, usually the hip joint protocol is almost always scanned bilaterally, only if you're redoing it for the same pacient and you know the history, but otherwise nobody asks for unilateral protocols.

Btw you can use in your video the positioning images from the Magnet's digital Manual, they're great so you don't need to take risky pictures inside the scanner room.

Thanks Bac!
We use the ufl 18 with spine coil. Might try the wrap next time though 👍

Hi Bac. Do you rotate the patient's leg medially as in XRay positioning, for MRI hip joint and MRI femur? Thanks in advance.

Interesting concept for better signal. Unfortunately my hospital's protocols have a bilateral view before the unilateral sequences.

I would always begin a single hip protocol with at least a STIR of the whole pelvis since non articular pathology is so common in hip presentations.

However then, I would move on to the narrow FOV single hip images you show nicely.

Could you acquire the whole pelvis using just the spine coil, then activate the flex to do the narrow FOV? to save repositioning

I'm curious how different body habitus' might make the different options to do unilateral hips look. It would be fun to test out sometime.

You should say MRI Technologist for your intro. Not MRI Radiographer
