How does the onion already sprout on the first day

Every time I try to do stuff like this the roots rot. I've done it with onions, scallions, sweet potato, chives ECT and propagated plants like succulents and inch plants to the same result. I don't put anything in the water and I change the water regularly but the roots always get moldy. Also I live on a ranch in a rural area so my water is from a well. I live in a good water state as well so there's no chlorine or anything like that in our water. I can't figure out why it happens but I can't get anything to propagate or keep any roots from getting moldy. If anyone has any advice I'd love to hear it

I have question in my mind about onion plant, if we plant an onion, how do we get another onion? What i see, we only get chives when plant an onion.

It wont last long.. the bulb usually will rot in the water.. plus, its impossible to grow fast with that..
