Chronic fissures are chronic. They heal then tear again chronically. For some reason some people keep having this issue. It’s been hypothetically caused by increased internal analysis sphincter with higher pressure that prevents the normal blood flow to the fissure site to heal it or by placing too much pressure lower in the sinus that pushes on the fissure. It’s a life long battle for some. Modifying diet to keep regular softer stools helps as does medications that increase topical blood flow like nitroglycerin or calcium channel blockers sits baths help so does creams that soothe the irritated area. You have to stay with these things long term. I’ve been using nifedipine ointment every day for years in spite the usual two months. If I stop within a few days we have a flare up or new fissure. This will most likely be a way of life for me. You can get Botox but it only works for a short time and repeated use isn’t a good idea. You can have several surgical treatments which may heal it but they can return in the same place or another. There is I easy fix for this just lots of things to try and stick with it. Wow. Thank you Doctor. I currently am experiencing a chronic anal fissure recovery now. Looking forward to long form video. You have been such a blessing to have this online community. No one around me has any idea what’s been going on. But seeing your positive attitude and hearing how we are all so similar. It takes the shame out of it. In the beginning I struggled so long because I was afraid of speaking out on what’s been going on. Since getting a fistula repair and LIS I have been getting better but still in the recovery for sure. Going to get a colonoscopy next Friday to make sure I don’t have any underlying conditions. Possible I caused the fissure due to longer time on toilet and way to much straining and wiping and heavy weights. But I never comment on videos like this. I just have seen so much help in the community now I feel I wanted to thank you Dr. Chung for establishing an air of lightness around this heavy topic. It’s not easy but we can do this!! Dr Chung help me heal my anal fissure and I am truly grateful. I had only one office visit and he referred me to a pelvic floor physical therapist. I didn't realize, but my pelvic floor was so weak, it causes the fissure. I know it feels like there is no hope, but there is one you figure out what the problem is.
