To me, it looks like in the process, Newegg might have an issue with the training of the person that was doing the inspection. If the return issue happened several times, where was the item initially checked? Were all these items checked by the same person? What checklist does the employee need to go through to look at the article? Another point would be to look at the resources available to the Q&A team. Do they have enough time to check the items correctly, do they have the proper checklist (or an older version)? All these show the importance of getting feedback from the employees for potential improvements and having management control from time to time if processes are followed as planned or not.
