A paramedic wouldn't give a patient CPAP if the patient has any of the following contraindications: altered mental status, meaning the patient is unable to follow commands or protect their airway; respiratory arrest, indicating the patient is not breathing or has inadequate respiratory effort; hypotension, where the systolic blood pressure is less than 90 mmHg; pneumothorax, which involves a collapsed lung; vomiting, due to the risk of aspiration; and upper GI bleeding, also because of the aspiration risk.

So hypotensive or they cannot obey commands are the two major contraindications? A bit confused by the phrasing in this short clip.
