You missed one: at the start of the game, in the room with the Dog prototypes drawing, there is a camera and a window with duct tape on it. The duct tape create a arrow pointing at nothing outside the window. However if you look through the camera at the same spot, you'll see a figure standing right where the arrow is pointing.

The combine dialogue is great in this game. There are so many lines. I love how if you run around and duck for cover sometimes they'll say something on the lines of "suspect is panicking"

Actually you can get the tiger mask a chapter earlier in "Jeff" it's in a room under a bucket in the first room you meet jeff. If you wear it and interact with the friendly npc he'll compliment your mask.
