You get this true ending by taking on Erlang Shen in his secret realm. According to Reddit you can do this by:

1. Unlocking four hidden maps: Old Guanyin Temple, Sand Country, Ziyun Mountain, and Bishui Cave
2. Collect all three artifacts in the Mandrill Buddha Head branch in Chapters 2, 3, 4, and 5
3. Defeat the clapping stone man in Huaguo Mountain, then defeat the giant BOSS on the path opposite the Water Curtain Cave and get the last artifact.
4. Return to the Little Western Heaven Pagoda, where Maitreya Buddha will open a new map for you.

For those confused about the ending. Basically the mc puts oj wukongs crown and then fully becomes him. The scenes here is basically the mc receiving all of wukongs memories and then the ending scene with him in the empty cave is the mc again looking into it and seeing and realizing all his comrades are gone so he leaves it to start his beef with the gods again. They might finish up the story properly with wukong beating the gods in a dlc or 2 because the next black myth game is suppose to be set in a different chinese mythological story featuring a different main character. Theres gonna be a series of black myth games each set in the world of different chinese mythological stories told. The first is based on the journey to the west novels.

At the end of it all it was a ploy by sun wukong to escape heavenly grip with the help of erlang by killing him and ensuring his new self to be born anew truly free.

Once the Destine One has beaten his former self (Wukong), he was given a choice:

Ending 1: To continue the indulgence in his previous believes and re-wear the Golden Circlet (signifies a form of self-imposed mental imprisonment, to continue to believe in the lies of the CCP), & to re-do his journey to the West again (The journey full of lies, deceit, peril, oppression & submission). Meaning that the "Destined One", the people, has not truly awaken to the truth yet.

Ending 2: (Also believed to be the true ending by many at this point): To reclaim his birth right through his own six relics (Sadindriya in Buddhism): Eye, Ear, Nose, Tongue, Body, & Will that "sees, hears, smells, tastes, feels, & WANTS" the truth. And truly be awaken and be himself, free himself from the shackles of the Golden Circlet (Indoctrination & mental bond), to fight the war against the Heavenly Court (CCP) once more. This time, with a clear heart and mind.

Many people here confusing the cut scene ending. The cutscene is the memory of journey to the west but reverse. The mc after defeating wukong gain the memory of wukong. And the ending of the cut scene is not that mc is starting to fight the celestial but it literally is the start of wukong story. That waterfall scene is when he first start fighting celestial kingdom, than Buddha intervens and tell him to cross his palm which he tries by pissing on his fingers and so on and on. People confusing this memory by thinking the end scene in memory is the mc
