My headcanon. It was both.

Anakin either got hurt in battle and played it off as an accident in the bathtub, or it was an accident in the bathtub, and he was embarrassed about it, so he said he got it in battle.

There was a second medium for showing he got his scar. The old Clone Wars animated. Before the clone wars movie or series we know now. Asajj first appeared there. Along with Grevious and Durge. And same, asajj was the one to give the scar
