This is symbolic to Shinji's character because he has two balls. If you count Shinji, and his balls, that makes three. There are three pilots for the EVAs: Asuka, Rei, and Shinji. Shinji enjoys being kicked in the balls because it symbolizes his inner self-destruction and the secret morbid curiosity of dragging both Asuka and Rei down with him so they can feel his pain and thus give him empathy. This also calls back to Adam and Lilith where it's stated that Adam through the use of excessive CBT can be awoken early and thus the Honkai Impact can begin which is an alternate version of the Third Impact.
