After 22 years of working the door, at many different bars, my favorite way of catching a fake ID is as follows.
I rarely see someone who has a fake come up to the door by themselves. They are typically the next to last, or last in the group.
All I do is ask them if the others if they are together. When they say yes, I ask their friends what their name is.
The friends rarely know what the name on their ID is.

One that you didn't mention is the feel and durability of the card. Fake IDs often feel different from the real thing. Also, they tend to be made of cheaper plastics, so a simple light bending of the card at the corner(but not enough to break it) will cause it to show whether it's fake or not.
Some places use a magnetic strip on the card's back. Running it through a reader also makes it a dead giveaway.

Used to work at several gas station. One story. Guy came up trying to buy beer. Asked him for ID. He gives me a manually typed Arizona ID that was still valid. I looked in the ID book and Arizona had computer typed IDs with the typewritten had been invalid for years. I grabbed the beer and threw the ID back with "get a better fake".
