After drilling the big hole I usually drill 2 smaller ones for the anchors and screws. Screws by themselves fall out easily.

Pull it out again, do the drilling probably! Scruw it back in, so it's flush.

Your square was backwards. Also you shouldn't align to the fixing plate as they are usually out of square. Alignment should be gauged by the hinge travel
