Bleeding uterine biopsy

💙ok we need to review some GOOD BASICS all!
‼️perfect timing bc @drheatherirobundamd kicked it off with her fab fun REEL yesterday about showing people all our instruments which works well with my last week’s “what is a speculum” and now “Oy! How are you biopysing my uterus?!?”
💙this is a skinny plastic straw like device that we use to “sample” (ie biopsy) the endometrial lining (aka inside your uterus—where the period blood sits!)
⁉️you may need one done for: heavy/irregular bleeding either (very common Especially during Peri menopause where it’s likely just hormonal changes but malignancy (a.k.a. cancer) always has to be ruled out…OR if you have ANY post menopausal bleeding…OR we occasionally will use this if you have had a miscarriage and a small amount of tissue is still inside..
💙prior to some brilliant human creating this—GYN’s only option to sample the uterus was a “d&c” (dilation & curettage) which is typically done with mild sedation or full blown (SAFE!!) anesthesia and less often as a “quick” office procedure
😩BUT is this uncomfortable (aka painful) you ask? WELL it depends BUT the SHORT answer is “yes But for the MAJORITY of women it is tolerable”. The LONGER answer is: your doc will likely tell you to take an NSAID (ibuprofen/naproxen) an hour beforehand—PLEASE LISTEN AND DONT ASSUME TYLENOL (aka acetominophen) is the same—it doesn’t help with the contraction type/inflammatory type pain like NSAIDS and that can make a HUGE difference.
🗣I ride the fine line between being HONEST and NOT being “scary” so I tell my patients “you will hate me for 10-30 seconds but IF we work together we can get this done quickly and efficiently and effectively” and I show them the device bc it does NOT look onerous—is it then painful? UMMMM YES—I’ve had it—BUT is it 10000% tolerable and QUICK (also YES)
❓NOW as always we will have patients who cannot have it done in the office for various anatomical reasons OR bc it really IS too painful and this does NOT MEAN THEY/YOU are wimps—it means each of us has a different pain threshold based on our past trauma, our actual nerve ending response, our anatomy and PROBABLY 10k other reasons we don’t even know! No shame
