Terminal Knee Extension (TKEs) to Work on Getting Your Knee Straight

Gaining full active terminal knee extension is one of the most important things after any knee injury or surgery, especially after an ACLr. While there are many more “fancy” drills I incorporate from time to time, the single most important thing I emphasize to get full knee extension is to actually ensure the patient is actively pushing into end range with every rep.

It’s extremely common to stop just shy of end range extension at the top of reps. Naturally end range is the “end” because it’s neurologically past whatever zone of ROM we are “comfortable” with. That being said, instead of spending an hour hanging out into passive extension or doing a more fancy drill to work terminal knee extension, just push and remind your clients to fully extend. Drive through their heel. Fully squeeze their quad. And feel a stretch at the top of EVERY REP. Think of the volume of reps in 1 session…that’s potentially hundreds of opportunities to drive home terminal knee extension. Don’t waste your reps!!

How do you get the quad to activate to do this exercise? Full left patella dislocation 3.5 months ago, arthroscopic surgery to remove scar tissue and cartilage 3 weeks ago. PT 3x weekly, stationary bike, massage, stim, static and moving cupping and my knee is slightly bent when standing leading to a bad limp. I can get full extension when sitting or laying on hard surface but can't fully extend when standing. I've tried this exercise many times and can't get the extension to push back on the band. Please help.

I'm still 10 degrees from total extension and so worried.
I'm 9 weeks post op from a RTKR.
Is there still hope for me ?? Please help if you can.
I'm already back to work on light duty, but I walk a lot at my job.

You've helped me a lot after my plica surgery after I got a bit of a plateau after week 3 post op. Thank you so much! One thing I'm a bit concerned is that I still have these swollen lumps around the incisions 6 weeks post op. Is that something normal?

Thanks for the clear video! I wonder if you know whether this will help resolve a runner's knee issue? I still seem to have very tight muscles, but as I took a long time off training to resolve it, I've lost a lot of strength. I've noticed my vastus medialis seems to be super tight and a bit tender at the knee, but I have full (albeit weaker) range of movement and no movement pain.
