Celiac disease is an autoimmune disorder where ingestion of gluten leads to damage in the small intestine. Gluten, a protein found in wheat, barley, and rye, triggers an immune response that damages the villi lining the small intestine, impairing nutrient absorption.

Presentation
Celiac disease can present with a wide range of gastrointestinal and systemic symptoms, which can vary in severity among individuals.

Signs and Symptoms
1. Gastrointestinal Symptoms:
- Chronic diarrhea: Persistent loose stools.
- Constipation: Difficulty in bowel movements.
- Bloating and gas: Swollen abdomen and excessive gas.
- Abdominal pain: Discomfort or cramps.

2. Systemic Symptoms:
- Fatigue: Constant tiredness due to malabsorption of nutrients.
- Weight loss: Unintended loss of weight despite normal eating habits.
- Iron-deficiency anemia: Due to poor iron absorption.
- Osteoporosis: Weakening of bones due to calcium and vitamin D malabsorption.

3. Dermatologic Symptoms:
- Dermatitis herpetiformis: Itchy, blistering skin rash, typically on elbows, knees, and buttocks.

Vitamin D and Celiac Disease
Vitamin D is crucial for bone health and immune function. In celiac disease, damage to the small intestine impairs the absorption of fat-soluble vitamins, including vitamin D. This deficiency can lead to:
- Bone density loss: Increased risk of osteoporosis and fractures.
- Muscle weakness: Due to insufficient calcium absorption, which vitamin D facilitates.
- Immune system impairment: Increased susceptibility to infections.

Diagnosis
Celiac disease is diagnosed through a combination of:
1. Serological Tests: Blood tests to detect celiac-specific antibodies, such as tTG-IgA and EMA-IgA.
2. Endoscopy and Biopsy: An endoscopic procedure to obtain a biopsy of the small intestine, which is examined for damage to the villi.

Treatment
The primary treatment for celiac disease is a strict, lifelong gluten-free diet. This helps in healing the intestine and preventing further damage. Additional treatments may include:
- Nutritional Supplements: To address deficiencies, especially in vitamins D and B12, iron, and calcium.
- Regular Monitoring: Follow-up with healthcare providers to monitor symptoms and nutritional status.

Vigilance
To be vigilant about celiac disease:
1. Awareness of Symptoms: Recognize and monitor the combination of gastrointestinal and systemic symptoms.
2. Consulting Healthcare Providers: Seek medical advice if symptoms persist or worsen.
3. Routine Blood Tests: Screening for celiac-specific antibodies can aid in early detection.
4. Vitamin D Levels: Regularly monitor vitamin D levels, especially if experiencing symptoms of deficiency.

👌👌👌 confirming the actual diagnosis is what every doctor should do. Instead of giving temporary solutions and looting people through pharma companies.

My childhood best friend has had this disease since birth and now she's 20. She never even had a tiny bit of gluten in her entire life and that's so hard to manage. She had to suppress her cravings as a little kid when everyone around her was having something gluten delicious. And I'm proud of her.

"Hello Doctor,
I'm a 24-year-old woman from a rural area in the UK/India. Despite my best efforts, I've been struggling to gain weight, and instead, I've been losing fat from my skin every month. I've also been experiencing stomach issues and am unable to consume packaged foods or caffeinated beverages. I eat homemade food only, but still, I'm not seeing any improvement. I'm concerned that I might have hyperthyroidism, as I've been feeling lethargic for the past six years. Could you please recommend some tests or examinations that I should undergo to determine the cause of my symptoms? I'd like to prepare myself for the next steps.

Thank you for your time and assistance."
