They realize that going to the bathroom as a time when you're "vulnerable", and they're on standby for threats. This goes the same for when they sit outside the bathroom door, or try to get in.
Now, if they're meowing, putting their paws under the door and trying to get your attention, usually exactly that. They want attention. Sometimes it's the want of attention to know you're safe while "vulnerable", but tbh? It's really hard to tell the difference between "GIVE ME ATTENTION!" and I AM WORRY PLS TALK TO ME OR OH OH OH!
