I still don’t understand this idea of ‘they’re too high-risk, therefore they’ll just have to die.’

What’s the worst that can happen with attempted retrieval, they bleed into the IVC? I’m sure the patient would have consented to it.

Cool case, nice presentation with lots of pearls! Really weird for such a big clot to develop. I noticed the Dbili was pretty elevated makes me think of an obstructive biliary process such as pancreatic malignancy or choledocholithiasis, was there ever any imaging showing the CBD diameter?

I would have advocated for anticoagulating at a lower dose. That clot (if it is a clot) is gigantic and can still expand
