Curious why the application for an Illinois has “other” as an option for the race of a newborn if the state will in turn deny your application because you selected “other” as the race of your child. Seems having it as an option on the application form is but a waste of ink and vehicle for confusion. We nearly lost a bid for insurance on our youngest because they had to wait such an extra long time for a birth certificate due to it being originally denied.

What provision of law do you use in the event that the Vital Statistics Act is inapplicable, you've already filed a petition for name correction, on your child's birth certificate, which was previously denied, and they're telling you to take it to Court?

What if your exes mom and the ex forge your information to put him on there and he’s not the father of your child. Filled out a correction form but nothing yet.

My son will be 3 next year. And I want him to carry my last name instead of the father. We are not married nor is the father involved. Will that still be possible to send the corrections to the state of Illinois?

Hello my friends, My father's middle name is incorrect on my birth certificate and he is born in Mexico. I have his birth certificate. Do I need to mail the original birth certificate or can I just make a copy of my dad's birth certificate with my copy machine and mail it out? Which one of the two?

My birth certificate's name was amended over 20 years ago by my parents in Illinois by voluntary acknowledgement of paternity, but they never corrected my social security record - it still has the original name. I've never had a form of photo / government ID like a driver's license, federally-compliant form of photo ID, or a passport.
My options are updating my social security name to what my birth certificate was changed to, or getting my birth certificate's name changed back to how it originally was.
I have old school records up to the 5th grade (never graduated highschool) that have me in my original birth name, some invokable dental records, idk. I'm fucked.
I was born in Illinois and lived there for about 18 years, , but I've been living in a different state for 4-5 years now. What should I do?

Hi My Grandmother Adopted me when I was 18 but none of knew I was born In IL and have had the same name for 56 years a new law for CDL Drivers came up in 2015 where I needed a copy of my Birth Certificate I could not get it because of different Last name I've had a License since I was 18 and a CDL Since 1993 I have proof that My Social Security Name has never changed since I was Born and My DMV Records showing My Name has always been the same PLEASE HELP Im without a job till I get this Mess fixed My Question: Is this a Correct a Birth Certificate or Something else Thamks

My son's father was there at his delivery and signed a paternity paper stating that he was the father, but we were only common law married in Oklahoma. We got his Social Security card and then applied for his birth certificate, but the hospital gave him MY name instead of his father's! We have fought with Oklahoma for decades and my son is now 27 years old and he cannot get a driver's license, a library card, a bank account or a job. I am 71 years old and terrified that if I die he will be totally helpless. He has Asperger's and we have no family here in California. Please, will somebody help him? I have tried everything I know how, and I live on Social Security. Please tell me what to do? Oklahoma had made me pay for his birth certificate twice and kept the money but will not give him his correct name. He is so despondent now that all he wants to do is sleep. I am scared to death he will just give up and die. Please, please help?!
