Becoming board certified is achieved by demonstrating a certain baseline level of competency in a medical specialty. Board certification is the highest certification a doctor can achieve in their specialty.
If the doctor is not board certified, it could be for one of two reasons.
It could mean that they simply do not have the necessary level of competence and knowledge to pass the board certification examinations. Alternatively, it could mean that they have not yet taken their board examinations.
In order to first qualify to take the board certification tests, a doctor must complete a residency training program. Once they have completed their program, they must then accumulate two years worth of actual clinical practice and experience before being allowed to sit for their examinations.
