I keep hearing how "trapped", "stuck" and "frustrated" pharmacists are who work in LTC facilities and SNF's and sadly Polypharmacy and Prescribing Cascades continue to run rampant under the supervision of clinicians.

Our system simply put...sucks....and we don't leverage pharmacists as the optimal deprescribing weapon that health care hasn't appreciated or realized. Worse yet, we as pharmacists HAVEN'T woken up to our Deprescribing powers and are unclear on how to MONETIZE this!
