My flappy do-dad has failed me on the rare occasion. Very dramatic coughing ensues.

I have MS and this is an hourly, if not constant threat for me, even just from saliva. It's torture, especially depending on what went down there.
I try not to eat in front of anyone but family. Sometimes I'm face down on the floor with my butt up, unable to move enough air to speak or keep my balance. I can only put my thumb up to let my family know I'm still conscious and the heimlich will not help me. After a minute or so, I can start inhaling and coughing again.
Some foods are extremely painful in my lungs and I avoid spicy foods for that reason only.
Right now I'm literally having PTSD symptoms when I see a tootsie roll, because I painfully got some down there and it made my upper lungs hurt for days. I love tootsie rolls, and I have a big bag of them that I hope to enjoy in a few days.
It's torture to have anything down there, and I've had pneumonia so many times I can't count. If someone water boards me, I will have to tell them what they want to hear.
I love hot wings, but it's been years since I could eat them.
Treat your flappy doodad with respect, cause you need it.

My flappy dudad is prone to spasm. Laryngeal spasms are a real problem for me. My husband and sister understand now (he’s an EMT, she’s a paramedic), and I handle them better than I first did. I know I have to be calm, relax, breathe through my nose, and wait for the spasm to pass. It’s important to avoid them by avoiding acidic foods and liquids, drinking or eating and talking at the same time, but the hardest are ones caused by coughing spells so I try to keep cough drops on hand.

The Tuesday tube! 😊
And then as your coughing fits begin, 4 people start asking you if you’re ok. As if you’re able to answer, right?

I once knew a family of three and they were all obese. The mother died from choking on food. Who would have ever guessed that would happen ?

Yeah, that's a real "intelligent" design. Let's put the pathway for breathing right next to the path where the food goes down. What could go wrong?

So what happens to the food or water that does end up in the wrong side

Thank you. That's a more precise picture of what I can see on the scan they did, although I do have the animated swallow test on a CD. Throat cancer, stage 3, spread to my glottis and elsewhere. Half of test fluid (1/2 tsp) went down, stopped and then went down my trachea. Tech immediately said, "Don't swallow again!" It was actually the worst advice because 6 or 9 months later, I couldn't swallow if I tried. I did figure out how to and could finally swallow water again. Have to hold shut my nostrils or else it comes right out my nose. No one explained why. I'm guessing cancer and/or radiation damage. And, yeah, I aspirate a lot. But I figured out how to pull out of it and breathe again. Been a close call numerous times, but I never said I'm the brightest bulb in the pack. Was it worth the risk to feel water in my throat again, sip coffee, taste food? Yup! I just can't go through life not tasting anthing. I sometimes wonder why they can't print out a new 3-D glottis and whatever else is damaged. Heh.
Edit: Found out 3 days ago 11/4 that there's no cancer left. Need to find out if there's any way to fix damed parts. And I didn't die 2.5 years ago as predicted by the ENT surgeon.

I like how food really physically bends the floppy doodad to close the breathing pipe. Nice.
