I am glad I listened to this! I like your synopsis of how SOAP notes have "evolved "! What a great way to further engage our patients in their care! The note as a "contract " with our patients for their care! Thanks again!

Reading back your notes to the patient has been something I started doing some time ago. There’s not a better way for patients to know you’re listening than by being an advocate for their story.

All the tips were good.

Let me ask you: how do you feel about taking notes during interviews? I always have but now that I’m a M1, a lot of the teaching physicians don’t.

I like your views on SOAP notes, especially your comments on plan to have a concurrence from patient/participant. Without their commitment to "plan" no treatment is likely to get best results.

Great topic that is often glossed over. In the UK we often use SBAR for written notes and verbal referrals and handovers - situation, background, assessement and recommendations of what needs to be done. It makes it succinct but includes the most important points for someone on call to know what needs doing and how urgently to do it. Great video as always.

This was helpful!: Especially the bit where we understsand that it's something to read back to patient if you should see them again.
-S: History
-O: Physical
-A: DDx (creschendo): MAY be X, but we should also consider Y; or even say something doesn't make sense
-Plan: specifically: patient has agreed to ... drink x 3x/ week, go to gym 4x month -- very specific

This is good stuff! I'm currently learning how to write SOAP notes and for the life of me I keep getting stuck on the Assessment and Plan portions. I feel like some other providers really write diatribes of information and I'm sitting here typing, "pt has the flu. They need to drink more water." LoL

Great video. How many hours do physicians spend with paper work per day? Do you do this notes in the end of your day or while talk to pacients? Is faster to dictate? Do you spend more time with pacients or doing paperwork? Thanks🙏
