I witnessed a loop about three years ago at my job. The other tech and I went to replace a computer and he didn't feel like moving the desk to see which cable came out the wall and which went to the switch. He placed the cable in the switch and left. About three hours later we went back to the main office and found supervisor troubleshooting the network. He gave us a time-frame when it may have occurred and I figured it out that a loop was created. It kicked our whole public works department offline until the cable was unplugged.
