You're making a systemic error here.

You're saying that analog records->EMR
This is true in the micro scale, but wrong in totality:
EMRs and EHRs are what we call the software systems that allow for the functionality of electronic record-keeping.
Not the actual individual files themselves - they no longer exist. This is at the core of the problem for the lack of interoperability.
