It should be remembered that many billionaires listed in the USA are not US nationals or hold several nationalities.
Federal dictats could very easily drive the most innovative entrepreneurs elsewhere and, in any case, personal fortunes are fluid and could be put to work outside the USA -- even considering that so much corporate money is already legally beyond the reach of big government. That is part of the nature of money in its various forms.
The USA is a failed state as far as health and social equity are concerned and to try to change course with a sudden lurch of the helm borne by the very wealthiest is extremely crude. The nation has had decades since WW2 to follow the models of the UK and some Nordic countries as well as Russia and China for that matter.
General medical and social equity is a mark of civilization and it requires the consent of all to pay taxes and contributions for provision according to need. The American way of making medical and social assistance available has hitherto been via commercial insurance and there is many a billionaire in that self-protecting industry and its sister 'big pharma'.
There have been periods in the history of capitalism when philanthropy has occurred but it does not thrive where executive government and party factions in Washington DC misjudge what would work best due to constant opposing stances and very big egos even within the traditional political parties.
