We can do genetic testing on samples to see how far back in time they diverged from a common ancestor. If the sample from mars is nearly 100% genetically identical with Earth's then we can be close to 100% certain that one hitched a ride from Earth to Mars and back.

Worked on this problem as an intern years back when Mars Sample Return was just getting introduced alongside Perseverance. If my memory serves correctly, I had to flag any chances that a unsterilized spec of “dirt” could reach Earth’s biosphere within 1 and 1, 000, 000 odds given the models I was dealing with. Pretty fun

The one fact that most people ignore when they imagine alien contagions is that any organism that grows on another planet would be so different genetically from Earth life that it probably could not survive for the inability to infect life here.

It's hardly ever in the movies but if there is life outside of this world that's even remotely similar to life here there is absolutely no telling what kinds of diseases could be spread that there are no immunities to.
